[ARCHIVED] Enrollment for dental, vision & flex spending accounts: Wednesday, Nov. 25 at noon

Please note that there has been no change in the benefits; and no increases in the premium rates for the Vision Plan or the three Dental Plans.
If you do not wish to make any changes to your dental or vision plans, you do not need to fill out any paperwork.
If you do plan to make a change to either your Dental Plan or your Vision Plan you will need to complete the Dental/Vision Benefit Election Form as well as the appropriate Enrollment Forms. Those forms as well as plan information can be found at: http://hampton.gov/index.aspx?nid=2574
If you wish to participate in either of the reimbursement accounts, you must complete the appropriate Benefit Election Form – even if you are participating this year. You must re-enroll in the reimbursement accounts each year. The Benefit Election Form as well as other information on the Flexible Spending Accounts can be found at: http://hampton.gov/index.aspx?nid=2955
REMINDER: Documentation is required to enroll dependents.
Please provide us with a Marriage License if you wish to cover your spouse, and a birth certificate, adoption papers, court ordered custody paperwork, etc. to cover a dependent child.
